Driving while intoxicated with alcohol may result in motor vehicle crashes, which represent a leading cause of death in the United States. An alcohol-induced brain dysfunction is hypothesized to be responsible for these crashes. The objective of the current study was to determine whether cognitive states or aspects of electroencephalographic (EEG) waveforms during the blackout period differentiate between two groups of subjects: (a) those with a crash history and (b) those with no crash history. The study participants consisted of 14 male drivers with a history of driving while intoxicated with alcohol and no crash history and 14 male drivers with no history of driving while intoxicated with alcohol and no crash history. After an EEG baseline, each subject performed two counterbalanced tasks during which they drove a simulated vehicle for a total of 6 min: a baseline driving condition followed by a 1-min blackout condition, and a baseline driving condition followed by a 6-min blackout condition. Electroencephalographic activity was sampled throughout the driving sessions with a 21-channel EEG. The single-trial averages of each channel were calculated for the baseline driving and blackout conditions and used in subsequent analyses. The critical comparison of interest was between the baseline driving and blackout conditions within the two subject groups. The present study found that mean spectral power of the subjects with a crash history was reduced compared with those with no crash history during the 3- to 4-Hz frequency band. These results suggest that subjects with a crash history have a decrease in activity in the hippocampus, a structure involved in memory, during the blackout period. No significant differences were found in the 14- to 20-Hz frequency band for the two subject groups.
