[Acute myeloid leukemia complicated by hemophagocytic lymphohistiocytosis--a case report]. A case of acute myeloid leukemia (AML) complicated by hemophagocytic lymphohistiocytosis (HLH) is reported. A 45-year-old woman was admitted to our hospital with a history of decreased appetite, general fatigue and anemia. She showed a high level of serum sIL-2R (sIL-2R) and lactate dehydrogenase. Since the bone marrow cells showed cellular hyperplasia with increased numbers of various granulocytes and megakaryocytes, we made a diagnosis of AML. However, only a few immature cells were seen in the bone marrow and no blast cells were identified. Since she showed marked elevation of serum sIL-2R, we diagnosed her with HLH. In the skin biopsy specimen, histiocytes with hemophagocytosis were found, which suggested the presence of hemophagocytosis. A regimen of etoposide, doxorubicin and cytarabine was started. She received a total of nine courses of chemotherapy. During each course, the sIL-2R level decreased, and the laboratory findings, including the number of bone marrow blasts, also decreased. The patient entered complete remission after the first cycle of chemotherapy and her sIL-2R level returned to the normal range. In patients with acute leukemia complicated by HLH, the usual treatments for leukemia such as induction and consolidation therapies are often difficult to use because of the severe pancytopenia caused by HLH, and in some cases, these patients are exposed to a risk of mortality. In the present case, we treated the acute leukemia and HLH at the same time. The treatment achieved complete remission of both leukemia and HLH.
