Completed a 4-week observership in outpatient cardiology with attending and fellow observers. Attended daily A.M rounds in Houston Methodist Hospital in Internal Medicine Department during which i attended the didactics which include board review conference, resident report and weekly once grand rounds. In Outpatient Service I actively engaged in taking history and physical examination and presented various cases to the attending and documenting them in EMR, later I participated in group discussion with other student case presentations. During these 4 weeks I have gained hands on clinic exposure in chronic hypertension, congestive heart failure, Valvular heart disease patients. Clinical Extern October 9 - November 10 th 2018. St Barnabas Medical Center and Modern Nephrology and Transplant LC, Livingston, NewJersey, USA. Completed a 4-week externship in the Inpatient Nephrology at St Barnabas Medical Center, NJ and outpatient Nephrology with attending. It was a hands on clinical experience. Attended In patient morning rounds with Dr.Bonomini at St Barnabas Medical Center in Nephrology ICU. Each day I saw 22-28 patients in the outpatient clinic. Responsibilities include taking a detailed history, performing a physical examination and documenting in the EMR - Cerner Got acquainted with Cerner and entered the patient notes. Every patient was then seen by Dr. Bonomini, during which we discussed the management plan. Many of the patients were on chronic dialysis and waiting for transplant, i went through the possible causes and counseled patients on health care maintenance, medication reconciliation, age related screenings, vaccination and importance of regular follow up. Learned to work as an integral member of a medical team in the management of Nephrology Patient.
